Is there anywhere for me to park?

There is paid parking on the street. Alternatively, there is free 3 hour parking in the Ormuz St carpark which is a few minutes walk away.
Are the splints / equipment / consumables e.g. wound dressings included in the cost of the appointment?

The splints / equipment / consumables are not included in the cost of the appointment for private patients and Medicare patients. The costs will be discussed with you during the appointment. Some health funds do refund some costs associated with custom made splinting. Please check with your health fund.
What should I wear to my appointment?

You should wear something comfortable for your appointment. To assess the area throroughly please be prepared to undress enough e.g. If your shoulder is being assessed please a singlet would be acceptable.
What do I need to bring to my first appointment?

Please bring your referral if you have one as well as any X-rays or scans. If you are using your private health insurance "extras", please bring your card. If you are on Workcover, please bring your claim number and medical certificate. If you are a DVA patient, please bring your DVA card with you.
Where is Revive Hand Therapy & Physiotherapy located?

We are located in Caloundra in Henzells Plaza. The address is shop 1A, Henzells Plaza 2, 45 Minchinton Street, Caloundra. The office is located on the upper level, near the Golden Dragon restaurant. You can access the upper level from Ormuz Avenue, via Trinity House (slope access), Minchinton Street (stairs) or from Bulcock Street (Wheelchair access).
